Prevention and Management of GI Bleeding and Luminal Perforation in 2021

Recognising, preventing and managing luminal perforation has become increasingly important for all endoscopists to understand and master. Perforation during endoscopy is fortunately infrequent but it is often a life altering and devastating experience for Endoscopist and patient alike.

Although iatrogenic endoscopic perforation can be instrument related, most commonly it is related to tissue resection (mostly polypectomy). For most endoscopists, this will occur in the colon and we will devote SIES series V to this aspect. Other luminal sites will be covered in the subsequent series. Fortunately, with the application of some simple rules this colonic catastrophe is easily averted or managed.

Dr Alan Barkun

Alan Barkun is director of the Therapeutic Endoscopy Third Tier Fellowship Training Program in the Division of Gastroenterology at McGill University and the McGill University Health Centre, Montréal, Canada. He is recipient of the DG Kinnear Chair in Gastroenterology at McGill University, holding a medical degree and an MSc in epidemiology and biostatistics from McGill University. Recipient of many national and international awards, Dr. Barkun has published over 700 peer-reviewed articles and abstracts, and has given over 600 international presentations on emerging digestive endoscopic technologies, with an emphasis on methodological, clinical and cost-effectiveness trials of treatments for upper gastrointestinal bleeding (UGIB), bilio-pancreatic diseases and colorectal cancer screening. He is also the current Chair of the colorectal Cancer screening implementation committee for the province of Quebec. Most recently he was awarded the Visiting Clinician Scientist Award in 2019 and the 2020 Distinguished Service Award by the Canadian Association of Gastroenterology.
